CAMPUS SECURITY
勒貊勛圖 complies with the Clery Act / Campus Security disclosures. All prospective students should review safety statistics, emergency procedures, and policy disclosures relevant to technical and healthcare campuses in Missouri.
This report includes statistics for the previous three years concerning reported crimes that occurred on campus; in certain off-campus buildings or property owned by 勒貊勛圖; and on public property within, or immediately adjacent to and accessible from, the campus.泭 The report also includes institutional policies concerning campus security, such as policies concerning sexual assault, and other matters.
Each year this report is made available to all students and staff.泭 All prospective students and employees may obtain a copy of this report from the administration office.

MIDWEST INSTITUTE ACADEMIC ADJUSTMENTS AND/OR AUXILIARY AIDS POLICY and PROCEDURES
In compliance with the Americans with Disabilities Act and institutional policy, 勒貊勛圖 offers academic adjustments, auxiliary aids, and alternate formats. Students in healthcare, vet tech, or technical programs can request accommodations by working with the Director of Education. 勒貊勛圖 will make such modifications to its academic requirements as are necessary to ensure we do not discriminate or have the effect of discriminating on the basis of disability against a qualified applicant or student with disabilities. Academic requirements 勒貊勛圖 can demonstrate are essential to the instruction being pursued by the student or to any directly related licensing requirement will not be regarded as discriminatory. Modifications may include changes in length of time permitted for completion of degree requirements, substitution of specific courses required for the completion of degree requirements and adaption of the manner of which specific courses are conducted, unless providing such accommodations would result in an undue burden or fundamentally alter the nature of the program, benefit, or service provided by the school. 勒貊勛圖 will provide educational auxiliary aids for students with impaired sensory, manual or speaking skills. Auxiliary aids may include taped texts, interpreters, readers, adapted classroom equipment or similar services. 勒貊勛圖 will not provide attendants, individually prescribed devices, readers for personal use or study or other devices or services of a personal nature
To request academic adjustments and/or auxiliary aids or service please contact the Director of Education. Individuals requesting an auxiliary aid or service, must complete an Academic Adjustments and/or Auxiliary Aids Request Form 30 days before the start of the program or as soon as practicable. Accommodations must be reasonable and supported by appropriate documentation. An interactive process between the student and the Director of Education will occur to determine whether and what academic adjustments and auxiliary aids should be provided. A written notification by the Program instructors notifying a student of the academic adjustments and auxiliary aids determined will be provided to the student.

勒貊勛圖 Family Educational Rights and Privacy Act (FERPA)
The Family Educational Rights and Privacy Act (FERPA) afford eligible students certain rights with respect to their education records.
Under FERPA (Family Educational Rights and Privacy Act), eligible students have rights to review or request amendment of their education records. This applies to all certificate programs at 勒貊勛圖 from medical assisting to veterinary technician training.
(An eligible student under FERPA is a student who is 18 years of age or older or who attends a postsecondary institution.)泭 These rights include:
The right to inspect and review the student’s education records within 45 days after the day 勒貊勛圖 receives a request for access.泭 A student should submit to the registrar, dean, head of the academic department, or other appropriate official, a written request that identifies the record(s) the student wishes to inspect.泭 The school official will make arrangements for access and notify the student of the time and place where the records may be inspected.泭 If the records are not maintained by the school official to whom the request was submitted, that official shall advise the student of the correct official to whom the request should be addressed.
The right to request the amendment of the students education records that the student believes is inaccurate, misleading, or otherwise in violation of the students privacy rights under FERPA.
A student who wishes to ask the school to amend a record should write the school official responsible for the record, clearly identify the part of the record the student wants to be changed, and specify why it should be changed.
If the school decides not to amend the record as requested, the school will notify the student in writing of the decision and the students right to a hearing re簫garding the request for amendment.泭 Additional information regarding the hearing procedures will be provided to the student when notified of the right to a hearing.
The right to provide written consent before the university discloses personally identifiable information (PII) from the student’s education records, except to the extent that FERPA authorizes disclosure without con簫sent.
The school discloses education records without a students prior written consent under the FERPA exception for disclosure to school officials with legitimate educational interests.泭 A school official is a person employed by 勒貊勛圖 in an administrative, supervisory, academic, research, or support staff position (including law en簫forcement unit personnel and health staff); a person serving on the board of trustees; or a student serving on an official committee, such as a disciplinary or grievance com簫mittee.泭 A school official also may include a volunteer or contractor outside of 勒貊勛圖 who performs an institutional service or function for which the school would otherwise use its own employees and who is under the direct control of the school with respect to the use and maintenance of PII from education records, such as an attorney, auditor, or collection agent or a student volunteering to assist another school official in performing his or her tasks.泭 A school official has a legitimate educational interest if the official needs to review an educa簫tion record in order to fulfill his or her professional responsibilities for 勒貊勛圖.
Upon request, the school also discloses education records without consent to offi簫cials of another school in which a student seeks or intends to enroll. The right to file a complaint with the U.S. Department of Education concerning alleged failures by 勒貊勛圖 to comply with the requirements of FERPA.泭 The name and address of the Office that administers FERPA is:

FERPA permits the disclosure of PII from students education records, without consent of the student, if the disclosure meets certain conditions found in 禮99.31 of the FERPA regulations.泭 Except for disclosures to school officials, disclosures related to some judicial orders or lawfully issued subpoenas, disclosures of directory information, and disclosures to the student, 禮99.32 of FERPA regulations requires the institution to record the disclosure.泭 Eligible students have a right to inspect and review the record of disclosures.泭 A postsecondary institution may disclose PII from the education records without obtaining the prior written consent of the student
To other school officials, including teachers, within the [School] whom the school has determined to have legitimate educational interests.泭 This includes contractors, consultants, volunteers, or other parties to whom the school has outsourced institutional services or functions, provided that the conditions listed in 禮99.31(a)(1)(i)(B)(1) – (a)(1)(i)(B)(2) are met. (禮99.31(a)(1))
To officials of another school where the student seeks or intends to enroll, or where the student is already enrolled if the disclosure is for purposes related to the students enrollment or transfer, subject to the requirements of 禮99.34.泭 (禮99.31(a)(2))
To authorized representatives of the U. S. Comptroller General, the U. S. Attorney General, the U.S. Secretary of Education, or State and local educational authorities, such as a State postsecondary authority that is responsible for supervising the universitys State-supported education programs.泭 Disclosures under this provision may be made, subject to the requirements of 禮99.35, in connection with an audit or evaluation of Federal- or State-supported education programs, or for the enforcement of or compliance with Federal legal requirements that relate to those programs.泭 These entities may make further disclosures of PII to outside entities that are designated by them as their authorized representatives to conduct any audit, evaluation, or enforcement or compliance activity on their behalf.泭 (禮禮99.31(a)(3) and 99.35)
In connection with financial aid for which the student has applied or which the student has received, if the information is necessary to determine eligibility for the aid, determine the amount of the aid, determine the conditions of the aid, or enforce the terms and conditions of the aid.泭 (禮99.31(a)(4))
To organizations conducting studies for, or on behalf of, the school, in order to:泭 (a) develop, validate, or administer predictive tests; (b) administer student aid programs; or (c) improve instruction.泭 (禮99.31(a)(6))
To accrediting organizations to carry out their accrediting functions.泭 ((禮99.31(a)(7))
To parents of an eligible student if the student is a dependent for IRS tax purposes.泭 (禮99.31(a)(8))
To comply with a judicial order or lawfully issued subpoena.泭 (禮99.31(a)(9))
To appropriate officials in connection with a health or safety emergency, subject to 禮99.36.泭 (禮99.31(a)(10))

To a victim of an alleged perpetrator of a crime of violence or a non-forcible sex offense, subject to the requirements of 禮99.39.泭 The disclosure may only include the final results of the disciplinary proceeding with respect to that alleged crime or offense, regardless of the finding.泭 (禮99.31(a)(13))
To the general public, the final results of a disciplinary proceeding, subject to the requirements of 禮99.39, if the school determines the student is an alleged perpetrator of a crime of violence or non-forcible sex offense and the student has committed a violation of the schools rules or policies with respect to the allegation made against him or her. To parents of a student regarding the students violation of any Federal, State, or local law, or of any rule or policy of the school, governing the use or possession of alcohol or a controlled substance if the school determines the student committed a disciplinary violation and the student is under the age of 21. Title IX
Title IX is a federal civil rights law that prohibits discrimination on the basis of sex in any program or activity at an educational institution that receives federal funding. Under Title IX, no student can be excluded from participating in, denied the benefits of, or be subjected to discrimination in any 勒貊勛圖 program or activity on the basis of his or her sex or gender.泭
The U.S. Department of Educations Office for Civil Rights administers and enforces Title IX.

勒貊勛圖 (the “College”) is committed to providing an educational, workplace, and public environment free from unlawful discrimination and harassment, including on the basis of race, color, national or ethnic origin, religion, sex, age, disability or perceived disability, sexual orientation, gender identity or expression, military or veteran status, or any other characteristic protected under applicable federal or state law. The College does not discriminate on any of the forgoing bases in its educational programs and services, hiring and employment, or any other opportunities and services it provides. The College likewise does not tolerate unlawful harassment, including sexual harassment and sexual misconduct, in any College programs or activities, on College property, or in any facility owned or controlled by the College.

How to File a Complaint
Complaints may be filed with the Title IX Coordinator through
- The online student complaint form泭
- Calling or emailing the泭Title IX administrator or coordinator
- An in-person visit with the Title IX coordinator
Anonymous complaints can be made via the online form.泭 While they are accepted, this type of complaint may affect reporting and resolution.
勒貊勛圖 takes complaints of sexual discrimination or harassment very seriously and encourages all students who feel they have experienced discrimination or harassment on the basis of their sex or gender to reach out to the College for assistance. However, students are not required to file a Title IX complaint with the College before filing a complaint with the U.S. Department of Educations Office of Civil Rights, which may be contacted as followings:

Reporting a Title IX Incident
Responsible 勒貊勛圖 employees must report all allegations of sexual misconduct that they receive to the Colleges Title IX Coordinators Office Not all reports will result in a formal investigation. However, all reports must be reviewed and addressed.
Investigating and Addressing Complaints
Title IX complaints are addressed through an investigation process. Complaints can be resolved formally or informally. The Title IX Coordinator initiates fact-finding regarding the complaint. Both parties to the complaint are offered an equal opportunity to present information, identify witnesses, have the support of an advisor, and provide information regarding allegations.
Training
All members of the Title IX team (Title IX Coordinators, Investigators, Decision-makers, and Advisors) receive annual training on the following topics:
- the definition of sexual harassment in 禮 106.30,
- the scope of the PCCs education program or activity
- Issues of relevance, including how to apply rape shield provisions
- the grievance process including hearings, appeals, and informal resolution processes
- how to serve impartially, including by avoiding prejudgment of the facts at issue, conflicts of interest, and bias
In addition, Title IX Coordinators receive training on:
- How to use the Title IX Notice and Communication Templates
In addition, Decision-makers receive training on:
- The technology to be used at a live hearing
- Issues of relevance of questions and evidence, including when questions and evidence about the complainants sexual predisposition or prior sexual behavior are not relevant
In addition, Investigators receive training on:
- Issues of relevance to create an investigative report that fairly summarizes relevant evidence, as set forth in paragraph (b)(5)(vii) of this section
- How to conduct a fair and neutral investigation
In addition, Advisors receive training on:
- The role of a Title IX Advisor and how they can serve their Advisees
All materials used to train the members of the Title IX team (Title IX Coordinators, Investigators, Decision-makers, and Advisors)
- do not rely on sex stereotypes, and promote impartial investigations and adjudications of formal complaints of sexual harassment
